  1. Special Cause problems: These types of problems are considered statistical anomalies, Asset breakdowns, sudden extreme variance in production output or quality levels.   2. Common cause: These types of problems often have a higher frequency of occurrence than Special Cause related issues. Controlling them will often mean having an influence in Quality level and Asset performance. Any organisation claiming an effective RCA process will be found to at least be on the way to controlling this variance. Solutions in this area are to be found in streamlining processes, changeover tactics, Operator care improvement etc.
  3. Catastrophic problems: This category contains all problems that have a catastrophic effect on the organisations business goals. They are often unexpected, Special Cause, but are to be avoided at all means because they may be catastrophic to your organisation. Even without an RCA program you should be containing these, but a structured approach to RCA will increase your chances significantly.
